1) INFORMED CONSENT

INFORMED CONSENT | IMPORTANT!

Please read, and agree below:

I agree to hold harmless Sara’s City Workout, Inc. dba SCW Fitness Education, SCW WIM, Inc. dba WATERinMOTION®, their subsidiaries, shareholders, directors, officers, employees, conference presenters, conference sponsors, staff (including staff assistants and convention staff), MANIA® sponsors, their respective agents, successors and assigns, from any and all liability whatsoever arising out of this event including, but not limited to: physical injuries, muscle strains, tears, pulls, broken bones, miscarriage, death, and any and all illness, or loss of personal property and income. I understand the risks involved with participating in this strenuous event and attest that I am in sound physical condition. I also understand that I may be videotaped, audio-recorded and/or photographed during this event, and Sara’s City Workout, Inc. dba SCW Fitness Education, SCW WIM, Inc. dba WATERinMOTION®, and their its affiliates and sponsors, may use any and all imagery before, during and after the event, both photos and videos, for any and all promotional and financial purposes. I further agree to all conditions of registration, including but not limited to, the No Refund Policy. SCW Fitness Education and its affiliates may give or rent your email address, mailing address and/or phone number(s) to other health and/or fitness related organizations with whom it has a trusted relationship and share common goals.

2) CANCELLATION POLICY

There are NO REFUNDS. Once a registration is submitted, it will be processed and can not be refunded for any reason; however a Letter of Credit or Transfer is available. A Letter of Credit is available for a $75 processing fee. All requests for Letters of Credit must be submitted in writing and include the reason for cancellation. All requests must be mailed or faxed to SCW or emailed to [email protected]. Letters of Credit are valid up to one year from the cancelled event. If the Letter of Credit is not used in one year, the amount is forfeited. A Transfer of registration to another party is available for a $75 processing fee. Phone requests can be taken for Transfers. Transfers can only be made if the new party is attending the same convention as the original attendee, and is not already a registered attendee. SCW will not grant Transfers or Letters of Credit after the convention date. SCW Fitness Education is not responsible for acts of God, nature, war, union or labor disputes or other events out of our control.
