After this 30 day period, please use the form below to request a copy of your MANIA® CEC/CEU form.  Please note that forms are available for download for previous attendees only.  There is a $35 fee for each download, please save your form in a secure place once received.  Along with a copy of your MANIA® CEC/CEU form you will also receive a Certificate of Attendance suitable for framing.
